Expanding Horizons: Collaboration and Integration Across Homelessness, Housing & Health – 7 Cities on Housing and Homelessness

• Wally Czech – on behalf of Community and Social Development, City of Lethbridge • Carola Cunningham – Executive Director, Ambrose Place • Doug Vincent – Provincial Director, Provincial Planning and Capacity Management, Alberta Health Services Facilitator: Sharon Blackwell, Director, Housing and Homeless Supports – South Region, Human Services, Government of Alberta Overview of Session: Homeless-serving agencies, health, and housing systems (among others), play a role in collaboratively building a system of care that seamlessly supports people to access appropriate services and permanent, safe, and stable housing. This session will explore integration as a concept, and how it is being articulated through various partnerships that have transcended traditional systemic boundaries and demonstrated successful outcomes in providing client-centred services to prevent people from becoming homeless, or to leave homelessness and achieve greater housing stability. As facilitator, Sharon Blackwell will link the presentations with Alberta Human Services’ shift to integrated service delivery, which reflects a commitment to transform service delivery from a programmatic to a client-centered approach.
